Catchment Area
If you do wish to register please download our registration form and GMS1. We also ask that you complete our Ethnicity form. Once completed please hand it into our reception.
You will need to bring 2 forms of identity with you before we can proceed with your registration. One a photographic ID and one proof of address. For a list of acceptable proofs please click proof of identity checklist.
You will be given an appointment with our Health Care Assistant for a new patient health check, during which we will take details of your medical history and current medication which will assist us while we are waiting for your medical records to arrive.
